Services and Rates
Each session is two hours in length. This time allows for evaluation, treatment, re-evaluation and movement re-education. Each session includes:
Rates $150 .................... two hours The first appointment is two hours. After the initial appointment you can decided which time works best for you. Two hours is recommended to allow time for evaluation, testing and treatment. Payment is due at time of service. Please inquire about package rates. Insurance I do not directly bill for health insurance. I can give you a receipt to submit to your insurance company in order for you to get reimbursed. You should check with your insurance company first to see if my services are covered. Some insurance companies require that you obtain a diagnostic code from your prescribing physician. I am unable to provide a diagnostic code. Please note that I am not part of any medical network and therefore, insurance coverage would be considered outside network coverage. |
